PRE-HOSPITAL EMERGENCY CARE

Ambulance Services

“The provision of skilled medical help at the scene of an accident, sports injury, medical emergency and during transport to the hospital”
As mentioned earlier in Home page and Introduction, Pre-hospital emergency care is equally important as the emergency department. The outcome of acutely sick or injured patient is dependent upon the care which patient received in pre-hospital setting.
The importance of Pre-hospital emergency care is apparent from “Trimodal death distribution” after an accident.
THE FIRST PEAK, Occurs from Seconds to Minutes of injuries e.g.; apnea due to severe brain or high spinal cord injury, rupture of heart and aorta.
Only prevention can reduce these deaths.
THE SECOND PEAK, Occurs within Minutes to several hours following injuries e.g; deaths due to subdural hematoma, hemo-pneumothorax, ruptured spleen, liver and pelvic fractures.
This Golden Hour (window of opportunity) of care after injury is characterized by rapid assessment and resuscitation …. by health care provider can have a positive impact on morbidity and mortality associated with injury.
THE THIRD PEAK, Occurs several days to weeks after initial injury. e.g.; death occurs due to Sepsis and multiorgan failure.
Care given in golden hour in prehospital area has an impact on the outcome of patient. So timely provision of skilled medical care in prehospital area cannot be underestimated. Government owned Rescue 1122 service was started in 2004 which has proved very effective in providing prehospital care in Punjab, Pakistan. This service has been extended to other provinces of Pakistan. This service is still far behind the similar services in developed countries.
The idea of this platform is to introduce Modern concepts of Prehospital Emergency Care which will be taught, presented and promoted among Healthcare staff, the students of Medical and Allied health Sciences and even common people.
Online teaching sessions, workshops and educational courses by Trained and Experienced colleagues will prove an important source of knowledge and skills for Healthcare staff in these areas.
This forum will help in replacing the old styled Pre-hospital Emergency Care with trained healthcare staff and modern equipment to provide improved Emergency Medical Care to acutely sick and injured patients as per International Standards.

SPORTS MEDICINE

Sports Medicine is also a new field which is nearly non existent in Pakistan. In contact sports many players are injured while playing. Moreover spectators also sustain injuries in the match or may have medical conditions which become aggravated during matches.
These matches should be covered by sufficient properly trained Medical staff (Sports Physicians, Sports Physiotherapists & Paramedics) to provide immediate medical treatment and then transferring the victims to the nearest hospital by well equipped ambulance service.
Unfortunately there is no concept of such services in Pakistan as there is no trained Medical staff (Sports Physicians, Sports Physiotherapist & Paramedics) because Sports Medicine has not been recognized as a specialty by “College of Physicians and Surgeons of Pakistan” (CPSP) or by “University of Health Sciences” (UHS) Lahore. Idea of introducing Sports Medicine at this platform is to highlight the importance of this specialty among Healthcare staff, the students of Medical and Physiotherapist.
